The Team named on this form agrees to meet all the
following terms and conditions. Failure to comply will result in a loss
of the Team Deposit.
- A Team consists of a minimum 6 skaters and 1 Goaltender.
- To Participate in the SPORTSPLEX HOCKEY LEAGUE for the __________________ season.
- Submit a $500.00 Non-Refundable Deposit to secure one spot in our League - The Deposit will be credited to the Team League Fees.
- Responsible for FULL PAYMENT of all Team Registration Fees by the Payment Deadlines set out by the League. (Post-dated cheques due at time of registration). Failure to meet ALL Payment Deadlines will result in the team being removed from the League.
- Submit COMPLETED Team Roster Form prior to first game. (All player names, phone numbers and signatures required).
- Submit Final Roster by League Deadline. (If any changes.)
- Teams are responsible for scheduling their own practice times
